Internship, June 12 th,2023 MY INTERNSHIP JOURNEY IN GENERAL MEDICINE I'm an intern, posted in general medicine department for 2 months including 2 weeks of psychiatry. First 2 weeks: Posted in Department of Psychiatry: Attended the OPD in department of psychiatry and observed various spectrum of psychiatric disorders in the patients. Studied symptomatology regarding various psychiatric disorder spectrums. I got to know about the ICD-10 Classification of psychiatry disorders and also DSM Classification of diseases. Done night duties for observation of psychiatry patients of withdrawal syndromes. And learnt how to manage them. Attended the De addiction centre visits and observed the various types of medical, behavioural therapy going on there. Learnt how to attend rounds for the patients and how to write repeats in the casesheets from post graduates. Typed, studied and submitted the discharge summaries of various patients under department of psychiatry. From this I learnt how

35 year old male came with C/o occasional palpitations and SOB since 3 months. June 04, 2023 This is an online elog documenting de-identified patient health data after taking his signed consent to enforce a greater patient centered learning. DEIDENTIFICATION - The privacy of the patient is being entirely conserved. No identifiers shall be revealed through out the piece of work whatsoever. CASE DISCUSSION - 35 year old male came with C/o occasional palpitations and SOB since 3 months. HOPI- Patient was apparently asymptomatic 3 months back, then started developing occasional palpitations, relieved on consuming alcohol, not a/w chest pain. He developed SOB since 3 months, Grade 2, insidious in onset, gradually progressive, no aggravating and relieving factors. C/o decreased appetite since 2 months. C/o generalised weakness since 10 days. No H/o orthopnea and PND. No C/o fever, decreased urine output, burning micturition, pedal edema. PAST HISTORY- Not a k/c/o DM, HTN, CVA, CAD, TB,

C/O Generalized weakness of lower limbs since 10 years Difficulty in swallowing since 15 days HOPI: Pt was apparently asymptomatic till 1999 then she delivered a baby(by c-section due to cord entanglement) she also stated that she started developed Generalized weakness ( due to low haemoglobin levels ) not associated with pain and was able to perform her daily activities Her husband also left as she couldn't perform her daily activities.Her son currently stays in the hostel She previously used to work as mandal officer. And in 2012 she stopped going to work because she started developing weakness insidious in onset , gradually progressive , associated with pain , Aggrevated with walking and relieved with rest. she could not walk for long distances and she managed to to perform daily activities In 2023 Jan she alleged had h/o slippage in bathroom following which she was normal for 5 days Next day she couldn't get up from bed which is sudden in onset and non progressiv
